I looked quickly the data sheet of this tube and based on that I would use the tube as triode connected (G2 and G3 connected to plate).
95 V supply voltage is quite low and does not allow big voltage swing, maybe max. 15 Vpp.
Try 22 k anode resistor and some 680...820 ohms at the cathode.
Fine tune the value of the cathode resistor so that the anode voltage is about 60 V.
Then the anode current is some 2 mA.
This tube has mu = ~25 when triode connected and Rp about 12k ( with 2 mA quiescent current).
The estimated gain is about 20 dB when the cathode resistor is not bypassed.

Looking at the datasheet http://www.mif.pg.gda.pl/homepages/frank/sheets/138/5/5702WB.pdf page 9 this tube in triode mode should work well with your power supplies. I assume your amp will be stereo, so you can run the heaters in series to connect it to the 12V supply.
