I am looking for unbalanced to balanced transformer like Lundhal, Sowter, etc. Could you recommend me a particular model & the way to connect the wires?

Do you need an input transformer? Just about any of good the 1:1 models will do, Jensen, Cinemag, Edcor, Lundhal, old Peerless, etc.
Connect one side of the primary to signal hot, the other to ground. Connect the wires of the secondary to your balanced circuit.

To have a gain of +3 dBs like the balanced signal how many turns ratio must have the step up transformer?

Turns ratio follows voltage ratio. 3dB = 20log(1.41), so that would be 1:1.41 turns ratio. 6dB would be twice the gain [20log(2)], which is 1:2. You will probably be more successful finding a 1:2 step up than a 1:1.41.
