Lafayette Phono/Reverb /Center channel amp

I got this for a Stereo systen I built back in the 1960's I would like to know if anybody could give me a model number for this unit. It has 4 tubes 6AU6, 6GW8, 6X4, 12BH7A. and is used as a Center channel amp, or a Reverb unit, or as a phono amp. I have searched the net and as close as I can get is the Lafayette Reverb II. Can Anybody HELP?

Did you purchase it from the Lafayette store on Euclid Ave., across from Olson Electronics?

No I got it in Canton, ohio on Cherry Ave.
