Has anyone ever tested a concertina phase splitter and had the signal not be out of phase at the cathode in comparison to the plate. I'm testing this now and the signals are not out of phase.

I assume you looking at an o'scope, are you looking at each signal separately (single trace) or at the same time (dual trace)? If you are looking at them separately you'll not see the difference due to triggering of the of the sweep. You'll need a dual trace to see it or at least that's the easiest way.
