Default Help with ECF80 spice model

I've got some trouble to turn those lines into a working LTspice model :

* ecf80t macro model

Could someone explain me how to proceed ? Thanks a lot

Btw, this model has been given by Bembel here : Need ECF80 / 6BL8 tube spice 3f4 model
