With a power and output transformer at right angles, should the choke be in line with the power, or the output?

Neither. You still have a right angle left.

Unfortunately, not in this case as space is pretty tight. Fortunately, this is for a guitar amp so a bit of noise is fine. But, if one must choose one way or the other, which does the least harm

Hmmm. I'd probably align the choke with the mains transformer.
