Default Diode blew, SE

After listening to the sweetest sounds I've ever heard from my TSE 45, for almost a year, diode 2 went south. Any ideas on why this would happen after so long? What else should I be on the lookout for? I found out when it blew a2 amp sloblow fuse.

Is this the FRED diode for the negative supply? Maybe not enough heat sinking.

Bias supply, -150V, so I'm guessing yes. Don't remember it calling for a heat sink, but I can sure add one, can't hurt.
