Sorry all.. Found a bunch of errors in the original. Namely B- missing from Output Stage. I was able to move things around and get some of the upper traces removed.
Updated is the fixed board. But its being updated as we speak..

Why not use the two layers equally, so you can optimise the length of the traces?
Generally I would clean up the PCB so your components are placed nicer, and maybe align some of the components to make the PCB nice to look at also.

Yes, just one question. Why are you using a double-sided pcb?
It should be fairly easy to design all your paths on a single side, at most using just two wire bypasses, close to the output transistors.
OK, another question: are you splitting your grounds between supply and input bars to be joined at your star ground?
