I had an amp die a few years ago.. anyone able to uncover why?

Hi guys.. only recently(over the last year or so) have i become interested in audio. A few years ago i had an amplifier that died(2 channel 60 watt/8 ohm sansui amp - was pretty old, probably late 80s.' one day i came home to find that a speaker terminal was shorted, the speaker fuses didnt blow but the mains fuse did. it had overheated. i no longer have the amp but does anyone have an idea why it died? could the output transistors/other transistors have internally fused themself?
Every time i replaced the fuse it blew it instantly
