And here is a photo of a populated board that should simplify component placement. The extra parts I have added are a multiturn trimpot on the input pair to adjust output offset and a Zobel network. The input stage supply is derived via Fast-Recovery Diodes and are separately decoupled.
(I could't get the full PCB design in the scanned output, no matter what I tried. Since, this is only to give those interested an idea, you could fill the missing tracks easily while designing your own board).
