Default Amp's output DC comes and goes, any suggestions?

Every 30-60 seconds, the output of the amp measures around 5VDC, then goes back to zero. Then it jumps back to 5VDC and back to zero again. This cycle continues indefinitely. Any ideas on what may be causing this? Thanks for all of the help! And this is not a gradual growth in voltage, it instantaneously jumps to about 5VDC.

without seeing a schematic, or the amp it's hard to say, but the first guess would be that the capacitor at the input of the feedback loop might be flakey...say shorting out intermittently..
