where does the +10db switch connect to? Or is it left open

Left open (not connected).
When switch is in the +10dB position, only R4 is in the path. In both other positions, another resistor is paralleled to R4, reducing its value. So, there are three possible values to the negative feedback. Hence, three different gains to this preamplifier. This kind of switching process prevents spikes in the output. Cheers, Max.
