Does anyone know what the bias voltage should be on a 404 S3? I currently have it set at 5.0mV across one of the emitter resistors on each channel.

What's the value of the emmiter resistors ? Make sure there is no load connected too.
Is it a BJT output or FET... and do you know the type of configuration, EF, CFP etc. 50 to 100 ma is a rough and ready current to aim for, in practice it won't make any subjective difference if it's off the mark. A lot depends on the thermal compensation too... if it's not so good then it may well be impossible to set and hold a figure anyway.
