Still not sure where the problem was but I removed the pcb from the heatsink, looked at it

it only takes a small misalignment and they can contact. If in doubt replace the insulator totally. If it is the mica type (looks transparent) covered in white thermal paste, remove all the old thermal paste and use fresh stuff - especially in MF amps as they love to run insanely hot
