I have no experience with SMPS.... could you elaborate a bit on the inductors, are they made, bought etc...? maybe some pictures... I think most people find those the first hurlde to overcome towards class D.

How does the PWM get power at start-up? Did you re-wound the driver xfmr? In original configuration the driver xfmr is used as a "proportional base drive", which runs for a second or two as a free-running oscillator at start-up, to generate secondary voltage for the PWM. Only after sufficient voltage has built up, does the Tl494 take over control.

This is seen by noticing the "extra" winding on the drvr xfmr to start the oscillator.
