| It looks like a small, non-powered bandpass woofer with speaker level in and speaker level out jacks on the bottom. |

I think cal has it. It will also have an XO in it. Mate it with a pair of small satelittle speakers and you have a sat.sub system. Note the sat impedance & efficiency will need to be in the ballpark of what it was originally mated with.

It looks very much like a passive Jamo sub from the early-mid 90s, similar to this one which is the active and a slightly later version.
