If I switch 400v rail in 100ns its 4kV/us accross isolation barrier not accounting for any transients.

Ahh...Yes, I hadn't thought of that.

The MOSFET source swings up and down 400V and carries the secondary side of isolator with it.

Then,...back to optical: Fairchild HPCL2611 has 10MHz bandwidth and logic level compatible, just right to feed the MOS driver.
It seems you can get logic gate optocouplers one order of magnitude faster than linear ones, if you don't care about analog levels.
