If you're talking about pure soundcards and not the card + break-out box combos, then I know of two:

1. CardD/CardD+ by Digital Audio Labs, ISA slot, for PC - AD1864.
2. SoundAccelerator II by Digidesign. NuBus slot, for Macintosh - AD1856 or PCM 56. NOS, without digital filter!!!

There might be some others, that I'm unaware of, but about these two I know for sure, 'cos I still have them. ;)

There also were several card + break-out box combos by Digidesign for NuBus Macs (early ProTools and SounTools systems) that used either AD1856, AD1861 or even PCM63, but I don't know any system that had PCM1704s. Industry swithed to delta-sigma DACs in the early 90's due to cost reasons.
Hope that helps.
