Aleph Topology - Power Follower instead Common Source Power output ?

By chance I have found the schema from the attachement.
The main difference to the Aleph output stage is the fact, that this stage works as power follower like that one about

but include the same features than the Aleph output stage concerning the enhancement of the efficiency.

The question is, how I can add a differential input amp to the power follower from the attached jpg file?

If I know this, there is automaticlly the possibility of choice about a select switch, to operate in two modes:
1) output stage as common source
2) output stage as power follower

Who can post the appropriate circuit topology?
