Nobody seems to have it. As I recall, it is simply an 8 switch DIP with
the loading resistor/cap right next to each switch element. The resistors
were RN55D with the value printed "side up" showing the three digit value
with multiplier - "4752" = 475 X 100 = 47.5K
With all switches open, I think the default is 47K with 100 pf or so, and
other common values were 1K and 100 ohms, but you can read the values
and see for yourself.
