You could do it on your own.
You just need a couple of phono plugs (I use Neutrik profi), the jack, some wire (I suggest CAT5 type from Belden, code 1634A, It's a twin twisted pair shielded) and probably some tape or expandable braid sleeve for a better finishing. The whole should cost less than 25 euro. You can save some money with a cheaper plug.
