Input jack connector for Huntington GA 10 Amp

I need to locate a part from a Huntington Guitar Amplifier GA 10 (MFG parts # AMP-G10).

I am looking for the input connector. It has 5 pins that fit onto the pc board. It houses a quarter inch jack input when the guitar chord is plugged into the amp.
There is no part number on the connector. It is made in China.

I can provide photos of the part I wish to replace. It has a crack in it. Thanks!

Are any of the pins in the four by the corner left unused? Because I sure would be grabbing one of these fender jacks from my drawer to see.
