Default Home meade design AMP prob's

Help with homemade amp design. I am self-taught and donít use schematics. I just draw out everything as it would be placed on the blank board. The red lines are hard solder wire. The small blues are jumpers soldered to the board. The long blue lines use pins and wires for each output and input needed.
First itís supposed to be an OPA2132 as a Preamp with its output connected to the INíS of a OPA4134. When testing all amps work great on their own but itís when you connect the outputs of the 2132 to the inputs of the 4134 it is nothing but elec noise. I tried a capacitor in line and 1K and 10K resister with no change at all. Any ideas why they work when connected to just one amp on both, but connecting the two does not work.
This picture would be looking down the top side of the amp. INís And OUTĒS with the wires so the placement on the picture is to just show how the L, R, Ground, pots, capís and resisters would be hooked up. Itís a dual RA1 design with one input but two pots so to two outlets can have their own volume

not easy to read from this picture......a schematic would be helpful.

lost ground connection between the two amps?
