A very common cause of tweeter failure is overdriving the amplifier into hard clipping. This produces high order harmonics at substantial power levels, and where does this high frequency energy go? Through the crossover network into the poor tweeter.

This is one very good reason for the additional expense and complexity of active crossover systems - the amplifier that drives the tweeter can be made small enough to reduce the risk of frying the tweeter.

For regular systems, ensuring adequate headroom in the amplifier (and preceding stages too) in order to eliminate the possibility of hard clipping, is highly recommended. Putting resistances in series with the volume control to limit the maximum gain is an easy fix.
