Horn Identification

I would be most grateful for any information regarding the Horn featured in the attached image file !

I am also wondering whether there are open source plans published for this rather nice looking Horn ?

Somewhere I saw this already, but I have to search in my PC....
I think "Planet 10" Dave has something similar.
