I don't know any serious class D projects but you can buy ready modules from two or three places.

I have tried the Hypex UcD180 and it seems to be everything the manufacturer says it is. Great sound, small size, little heat, easy to connect and low cost. What more can you ask.
I also use the ICE modules (I am an OEM) and they also work very well but the Hypex ones are easier to get.
