Default Feedback on my small 12v classD amp


Can someone with more experience than me criticize my lay out skills

It's a 12V class D amplifier based on the MAX9744. Dimensions are 29x25.5mm

Main questions:
-Can audio gnd be referenced to the gnd plane? If not; where do I connect it to.
-Do all pins with the same function have to be connected? The two pvdd pins at the bottom of the IC cause a big cut in the gnd plane
-How do your normally connect all the via's to a heatsink, like on the 1117 regulator on my board. Is there a better way than how I did it?

And of course the things I don't see / understand yet.

I noticed that the bias cap wasn't connected after I routed the whole board. First I wanted some input on the general layout before doing it all again.
