Frequenct failure of HIP4080 Class D H bridge

We have developed Class D amplifier with H bridge configuration isng HIP4080. IRF230 from IR are MOSFET used in circuit. We have to give 60W power for 50Hz sine wave to drive a linear motor. As linearity and distortion are critical, we have used close loop circuit as shown in application note of HIP4080. Our problem is HIP4080 is failing frequently. Our observation is that when we change sine wave amplitude from function generator to change output power, at that instant at some of time it fails. If full power is set, IC does not fail but gets warm only. When we are increasing/decreasing sine wave amplityde, all of sudden it fails. Can anyone guide us ?
 
The HIP thing doesn't look very much transient immune. It is not to blame as datasheet specifies that.
This fact in conjuction with using sense resistors and mosfet in high parasitic inductance package could be a reason.
They may be others though, including layout. Could you provide more data on the circuit?
