I have check your eagle file just ago,For my experience you should attention four key point.
1. TAS 5630 can deliver out so large power,output inductor and capacitor should big enough,common parts used in lower power class D amp not perfect. 19A ripple current for inductor and capacitor. I think your selected inductor and capacitor not perfect. 2.Pvdd should added at middle line of four energy electric capacitor,otherwise peak swith current not balance,maybe one channel output MOSFET burnt at heavy loader . 3,All the power pcb line and output large current route should smart and smoothness transition zone.forbid route width saltation. 4,Buffer OP amp as near as tas 5630,not suggest divided two board. other key point you have to attention is heatsink design.but it easy. Thanks for your advice, I will increase the track width for the power line and output line. The capacitor I selected is 1000uf/63V type, and the inductor I planed for drum core.
I am planning for my sub amp, same input for both channels |

My major concern is the large slot bisecting the ground on the bottom layer of the board.
For switching circuits it is highly recomended to have a contiguous GND plane with no slots. This could cause some serious EMC problems, and may stop the circuit working at the optium.
