I just completed 4 of these. It was loosely based on an Elektor design, but I changed it to suit my needs

Bottom. You need to mirror it, I dunno how to mirror an image from an export in Eagle.
Also, I didn't include the 22k muting resistor to -V in the design, so I soldered it under the board between pins 4 and 8.
