I've seen quite a few different designs online (not that they differ a whole lot )
I think I've even seen a circuit for controlling large motors with them!
better off controlling motors with an H-Bridge -- since the transistors don't operate through the linear portion of the curve you don't dissipate that much heat -- both International Rectifier and National have some great ready-built H-Bridge chips.
LM12's show up on EBay every once in a while, as do the APEX high voltage opamps -- you just have to be looking (and I don't have the time.)
