Angry 4v dc offset

How can I get rid of a 4v dc offset on a lm3886?
I have built a 4 channel amp with the lm3886s, and only one puts out dc. When I give it a signal and turn it up, it works, but it still puts out 4v dc.
All the chips are connected the same way, and 3 of them work correctly.
What can I do to get rid of the offset?

if everything else is the same, it may be a bad chip. try to change it.
try to put some of the other 3 chips in it's place for confirmation.

Watch out for small pieces of metal connecting things that shouldn't be connected, and look out for bad solder joints.

I just took apart an old amp that had +V on the output, and looking closer, found that a *TINY* piece of copper wire had landed between the +in and +V rails while I was soldering it, botching the whole thing...

*sigh*
