TAS5630 EVM always shows CLIP Light On

I have build TAS5630 DKD2EVM and TAS5630PHDR EVM .

I have two issue with two EVMS.
Issue 1.

There is no audio output and Clip light never goes away. All voltage seems to be OK. Not having detailed voltage reference for the TAS5630.

Issue 2.

There is Audio Output but after 1 or two minutes of working the audio starts to crack like some type loose connection.. I resouldered the IC but still the same result..


any help will be appreciated.
