Not anymore available. But maybe he can help you, if you contact him. Sometimes you stumble accross a copy of these on eBay. If none of that works, you can always use four normal PCBs and connect them together bridged and paralleled. You only need to add the load-sharing resistors off board, and match resistors on the PCB according to the application note.
