Need a part # for D5 of the driver board. This driver board has the 4 IR21844S IC's. The cathode of D5 ties to pin 13, anode ties to pin 7.

It's is the same as D1, D6 and D3, looking at one right now. I have 2 driver boards here at the moment. One has X355, another has X378
The reference design/demo board from IR lists this diode as a 600v 1A fast recovery diode and lists the MURS120DICT-ND specifically. The MURS160 would likely be the right part because the 120 is a 200v diode.
The driver board I have here (DWM3640 - similar to the 4-IC board) has an SMA package diode. The MURS160 is in an SMB package which is a bit wider.
The US1J is in the SMA package and may be a better fit on the driver boards.
Bear in mind that I've never had to buy replacements for these so I don't know if the components listed here will work.

The sanken part is 300V 2A fast recovery. I can go with the wider bodied part, plenty of room.

Replaced the bad diode, found an op amp pulling the +15V supply down - replaced that one.
Now the amp shuts down right after the second relay closes. No protect light, just a complete shutdown.

In the time before the relays engage, look at the signal on the output side of the large filter inductors. Do you see audio or do you see a spike of DC?
If you have DC, that's why the relays aren't engaging. The problem is likely on the driver board but you should confirm that all regulators are producing the correct output voltage and that those voltages are reaching the driver board pins.

Voltages seem fine with the driver card removed. The two fets that tie to U5 pin 12 seem to fail constantly. I get no oscillation on the gates of any output fets.
That would tend to indicate that there is an open circuit between that pin and the FETs that's letting the gate voltage float. The connection could be intermittent.
All gates are driven to the negative rail with no oscillation. I should note those fets fail when I have the protection circuit removed, D206. Otherwise the amp will go straight into protect.
