Default Rockford Fosgate Punch 150

I got another one of theese in ..

I replaced the outputs in this amp..

The channel with the arrow pointing to it still has very distorted output..

Any ideas?

Defective op-amp, defective driver transistor, loose connections on the fuse holders or a blown fuse.
