Pin 4 should have -15v.

I'm assuming that this is IC1.

Pin 4 goes back to the negative 15v regulator. ZD2 may be shorted.

Zd2 was shorted. Random failure?

Will the V- always be the same as the V+ for an op amp? Meaning if V+ is +12 can V- be -6v? Or does it depend on where and how its used?

Thanks for your help again.

When run from split supplies, they will be close but there's always a slight difference.

Not all op-amps are run from split supplies. Some have their negative power supply terminal connected to ground and will have supply voltage on the positive supply terminal only. This is common in head units, budget signal processors and class D amplifiers that use the HIP4080 driver IC.
