Ok, well it is mostly fixed. Turns out it was the RCAs after all. After some tinkering, i got the headphones to rca wire to work. Since the HU was new, i took it back to Best Buy, argued with the installer, finally got an exchange, and it works (sort of). The SW output still has little sound, but the rear works great. Id like the adjustability of the SW output, but I am content as-is. Thanks.

If the subwoofer output from this head unit has also failed, your amp may have an intermittently shorted transformer that's damaging the head units. If the RCA outputs that you're currently using fail, you'll need to check the amp more thoroughly.
