With all power semiconductors clamped tightly to the sink and a 10 amp fuse in the B+ line, power the amp up and twist/push/pull on the transformer to see if it blows the fuse. If you have a current limiter (headlamp or large 2 ohm resistor), that would be better than the fuse. You'd have to monitor the voltage across the resistor with a meter to see if there are any changes in current draw.
 
At the moment it won't even power up and it is getting 5v out the chip as it should so it's the +12 side that is not working properly I'd say. I dunno, about to chunk it out the window and forget it. Not worth it.

It works then don't, then works, then don't. Now nothing at all.
