If you call Rockford fosgate at 1-800-366-2349 and talk to tech support and give them the serial number on the white sticker on the circuit board they will be able to tell you what model it is

Perry is right, looks like a 500x, but the 400.4,400a4 is the same. With that case, it should be a 500x or 400.4.
