Sadly I'm pretty sure I've run across this application note, but no longer have any of my Burr Brown books, and was as unsuccessful as you finding it on line.
At one time I had an extensive library, 50kg+ of semiconductor manufacturer databooks, but with the advent of the internet several decades ago I decided after numerous moves to get rid of them.

I have a copy of Burr Brown's Databook from 1995. Whilst both devices are detailed, there is no reference to a RIAA PhonoAmp in the list of Application Bulletins so I can't provide the number of such. The details you require may be contained in a bulletin whose scope is wider than RIAA specific.
