You could do a lot worse than the circuit National Semiconductor has in their app note for their new LME opamps.

Check out the Hagerman Technology Bugle preamp kit; those are available on eBay for reasonable prices. Reviews suggest that they perform very well.
http://www.hagtech.com/bugle.html There's enough info there to build one from scratch if you wanted. If you listen to house and techno music, you could build a classic DJ mixer phono stage: the schematic for the UREI 1620 can be found online. There's some images online of the 1622 phono card which clearly show the layout. |

I strongly recommend against "passive eq" with flat op amp gain stages in RIAA preamps as shown in the national datasheet:
