3.5mm jack to rca - what conductors?

Just got a Chord Mojo dac and its outputs are 3.5mm headphone jacks, it can go into line level mode, so I thought I'd put together some jack to rca cables for myself. So which conductors should I use bearing in mind that both signals and both grounds have to fit into a pretty small adapter.

I'm open to buying a fancier big-holed jack adapter if they exist.
