Don't worry over the choice of opamp. The 4558 can be used or replaced by most common dual types such as NE5532, OPA2134 etc etc.
There are loads of tone control circuits around if you search although I couldn't just locate one with a PCB design... not easy to build ones anyway. Audio Designs With Opamps -2 3 Band Graphic Equalizer
