Buffalo II dac failure modes

Hi,
I have a Buffalo DAC II module that is constantly in mute LED on state.I was trying to figure out what all can go wrong but realized there is no dedicated thread listing failure modes. Since it has been few years since the board is out I am sure this might help many others too.
So my system configuration is below
DIYINHK USB to I2S module providing I2S input using 3-4cm Teflon cables to Buffalo. Power supply is 5.03VDC from SALAS REFLKTOR.
Buffalo positive and ground feeding my passive stepped attenuator of 10Kohm and then to power amp.
Can someone help me with a step by step trouble shooting?

Well not that much to check. Vin 5.00 to 5.25.

If fitted with tridens:
- one Trident @ 1.2V
- two Tridents each @3.3v
- AVCC @3.6v (has no influence on source lock)

Easy to check the board with spdif input. You can wire it directly. Just remember to flip the comparator switch.

Question, who says that it's the Buffalo that's failing? It could be the input board.

Can you show your I2S connections?

Do you have another source (I2S or S/PDIF) you can test with?

Have you set the S/PDIF switch to OFF?
